Annual Dental Membership Plan
Membership Benefits per Enrollee:
- Two preventative cleanings
- Two comprehensive exams
- One set of bitewing x-rays
- One fluoride treatment
- One panaromic x-ray every three years (full mouth x-ray)
- One limited (problem focused) exam and single x-ray per year
- 15% discount on all basic and major dental work including sealants

Terms and Limitations
- Annual enrollment fees are processed with the first monthly payment of the enrollment or renewal
- Patients will be charged for the full years membership if membership is cancelled mid-year
- Family members can enroll at different times of the year. However, the end of the benefit year will remain the same for all family members: plan expiration will be 365 days from the last family members’s enrollment date. Family members include parents, legal guardians, and children.
- Any benefits not used in enrollment year cannot be carried over to the the following year
- Family members cannot be substituted for other family members. Enrollees’ names must be stated at the time of enrollment. Children are covered upt to Age 26. This plan is non-transferable.
- This is an In-House Discount Plan and is NOT dental insurance. It cannot be combined with any other discout or dental insurance.
- Membership is only good at the office of Smiles on Broadway Dental Care located at 5442 Watkins Drive, Jackson, MS, 39206. Treatment outside our office is not included in the Smiles on Broadway Dental Care Dental Membership Plan.
- Plan enrollment is valid for 2023 and is subject to change thereafter.
- Plans will authomatically be renewed at the end of the enrollment year unless participant calls to cancel.
- To cancel the policy and receive a refund for the membership fees, policies must be cancelled with thirty(30) days after the purchase of the plan.
Coverage for the Smiles on Broadway Membership plan is as outlined above. Members have up to 30 days after enrollment to cancel the policy. Plans may be cancelled and reimbursed if services have not been provided prior to the 30-day deadline. Services will not be rendered if payment has not been received prior to the appointment date.
